New Students



 The following documents are required prior to your child's first day at school.

• Completed and signed ENROLLMENT APPLICATION FORM/RE-ENROLLMENT FORM.
• Copy of your child's BIRTH CERTIFICATE
• Completed and signed STUDENT'S MEDICAL REPORT from student's attending doctor.
• Copy of your child's CURRENT IMMUNIZATION RECORD
• Completed child's PERSONAL RECORD
• Completed and signed POLICY AGREEMENT page of Parent's Handbook

Returning Students

• Re-enrollment form
• Updated medical Form and immunization record
• Completed and signed Policy Agreement
